Purpose of the Social Health System Model
By addressing the conditions of the environment around us, both our health and what affects our health are better understood! The Social Health System Model will be used to evaluate East Cleveland residents based on the conditions of the community, and to provide them with access to health services so that they are more able to live a healthy lifestyle.

Social Determinants of Health
The Social Health System Model focuses heavily on addressing the social determinants of health, or the factors of our immediate surroundings that affect our health, such as access to health care and available food sources. If, for example, we live in a community in which fast food is the only available food source and health care is not available, our health will be negatively affected.
