About Us

Physical health, behavioral health and social service agencies are working with community resources to identify, analyze and focus on what affects the health of East Clevelanders. 

Who?

The Social Health System Model (SHSM) team is made up of individuals and organizations who are involved in the health and care of East Cleveland residents.

 What?

The Social Health System Model seeks to connect public health, social services, behavioral health and physical health to better serve residents and to create a healthier community.

Why?

Complex issues within the nation’s current health system exist that disproportionately affect the health and wellbeing of low-income individuals. The proposed Social Health System Model addresses a major underlying theme in all of public health – poor connectivity and fragmentation within health care. This research provides an ideal opportunity to strengthen the role and infrastructure of the public health system and address the social determinants of health.

How?

Development of the Social Health System Model has been funded by the Kresge Foundation as part of the Safety Net Enhancement Initiative.  Success with this planning grant will result in the ability to implement solutions through future funding.  This plan must be submitted to the Kresge Foundation by September 2010.
